Forms

Overview

Forms are documents that are available to the public and internal users for submission. The form type identifies the purpose of the form. Form types include applications, service requests, permit change forms (modifications, renewals, transfers, and terminations), complaints, and schedules.

Form submissions are the primary way to initiate a work activity in nVIRO. Forms are designed and managed by internal users with specific administrative rights.

To view all available forms, navigate to Admin > Forms.

Form Statuses

The Status column on the Forms page includes up to three statuses.

If a draft version of a form exists (which is always the most recent version), a Draft label is shown. If there is an earlier version behind the draft, the status of that version is also displayed (Published or Inactive).

In addition, the validation status of each of the displayed versions is shown. Forms with an Invalid status are not available for users to fill out.

Adding a Form

To add a new form:

  • Click the Add New Form button to open the Add a New Form dialog.

  • In the Create Form From field, select the source from which the new form will be created. The options are:

    • New Blank Form: Creates a new, blank form.

    • Import from File: Opens the Forms page in nFORM, where an Import Form button is available. For more information on this method, see the next section.

    • Copy an Existing Form: Creates a copy of an existing form.

  • Select the New Form Use Type. The options are:

    • Standard Form: Creates a new form that is not a component form.

    • Component Form: Creates a component form only. For more information on component forms, see the Configuring Program Component Forms page.

  • If “Copy an Existing Form” was selected in the Create Form From field, either the Standard Source Form or Component Source Form field is enabled, depending on the selection in the New Form Use Type field. Select the form to copy.

  • Click the Create Form button. The Form Details page opens for the newly created form, along with the nFORM Design Wizard in a new browser tab.

Exporting and Importing a Form

A new form can also be created by exporting a form from one environment (for example, UAT or Test) and importing it into another (for example, Production).

This method always creates a new form in the target environment. An imported form cannot overwrite an existing form.

To export and import a form:

  • In the source environment, navigate to the Forms page (Admin > Forms), click the Actions button (⋮) next to the form, and select the Design option to open the nFORM Form Designer.

  • In the nFORM Form Designer, click the More Options button (⋮), then select the Export Form option to download the form as a .export file.

  • In the target environment, navigate to the Forms page (Admin > Forms), and click the Add New Form button.

  • In the Add a New Form dialog, select “Import from File” from the Create Form From field.

  • Click the Click Here link to open the nFORM Forms Manager.

  • In the nFORM Forms Manager, click the Import Form button in the upper right of the page.

  • In the Import New Form dialog, click the Choose File button, browse for the .export file to import, and then select the relevant organization from the Select the organization the new form will be associated with field.

  • Click the Import button.

    • A new form is created with an initial tag consisting of a randomly generated number (for example, 3423423662). Be sure to change the form tag to a meaningful, unique value consistent with how other forms are tagged (for example, AIR_NEW_TITLEV_APP). Although the tag does not affect functionality, it is important for organization and for identifying forms easily.

    • Text is automatically appended to the form name to indicate it was imported; edit the form name to remove this text. At this point, the form can be edited and published just like any other form.

    • It may take a few minutes for the form to appear on the Forms page in nVIRO. Navigating elsewhere in the application or logging out and back in may be necessary before the form appears.
